The Framingham cohort set the baseline: Researchers tracked more than 5,000 people for 30 years, all free of heart disease when they enrolled. The higher someone’s heart rate at their check-ups, the more likely they were to die during the study — at every age, in both men and women.

A 2024 analysis pooled Framingham with Paris Prospective I and Whitehall I to ask what happens when the number changes. Among Paris Prospective men, each 10 bpm increase over five years was associated with a 20% higher mortality hazard. Over eight years in Framingham, the figures were 13% for men and 9% for women.

People with better cardiovascular fitness tend to have lower heart rates and live longer. However, resting heart rate isn’t just a fitness score. It also reflects stress, sleep, and how your nervous system regulates your heart. Consider the Copenhagen Male Study: It gave 2,798 men a cycling test, then followed them for 16 years. Take two men who scored the same, equally fit. The one with the higher resting heart rate was more likely to die during the study, by about 16% per 10 bpm.
